Individual Infectious Disease Case Reporting
Healthcare providers must report all suspected and confirmed infectious disease cases on DPHHS’s Communicable Disease Reporting list. Fill out the appropriate form (see list below) and submit via the secure file transfer. If you need password assistance please call 406.258.3896.

Infectious Disease Outbreak Reporting
Several types of facilities are required to report when an outbreak occurs at their facility. This includes healthcare settings such as long term care facilities, hospitals, outpatient clinics, and surgery centers. It also includes community settings such as assisted living facilities, group homes, shelters, detention centers, schools, daycares, religious organizations, and community events.
There are two options for reporting:

Facilities can fill out the Outbreak Line List Excel spreadsheet below and submit them to our secure file drop.
